Did You Recently Become Married or Divorced? 1/25/2008

Did you recently become married and changed your name to your spouse’s?  Or, did you recently become divorced and changed your name to your former name?  If the answer is yes, then you should complete SS-5 and file it with the Social Security Administration.  The form is available on the Social Security Administration website.  We have provided the link below.  Failing to notify the Social Security Administration may cause problems and delays when you file your tax return.  The IRS may not be able to match tax payments you made if your name and social security number do not match the records on file.

When you have completed the Form SS-5, take it with you to your local Social Security Office.  You will also need to bring documentation such as your driver’s license to identify yourself.
